Understanding Shoulder Pain

The shoulder is a ball-and-socket joint with an unusually shallow socket — a design that allows an extraordinary range of motion but makes it inherently dependent on the surrounding muscles, tendons, and ligaments for stability. The rotator cuff, a group of four muscles and their tendons (supraspinatus, infraspinatus, teres minor, and subscapularis), is the primary stabilizing structure of the shoulder — and the most commonly injured.

Shoulder pain ranges from mild aching with overhead movement to severe, constant pain that disrupts sleep, limits daily tasks, and prevents athletic activity. It is one of the most common musculoskeletal complaints in clinical practice, and one of the most frequently overtreated — with surgery recommended in many cases where conservative care would produce equivalent or superior outcomes.

Common Causes

Rotator Cuff Tendinopathy

Degeneration or inflammation of the rotator cuff tendons — the most common cause of shoulder pain. Produces aching with overhead movement, weakness, and pain at rest in more severe cases.

Rotator Cuff Tear

Partial or full-thickness tears of one or more rotator cuff tendons — causing significant weakness, loss of range of motion, and pain. Partial tears often respond well to conservative care without surgery.

Shoulder Impingement

Compression of the rotator cuff tendons between the head of the humerus and the acromion — producing pain with overhead movements and a characteristic arc of pain at shoulder height.

Frozen Shoulder (Adhesive Capsulitis)

Progressive fibrosis and thickening of the shoulder joint capsule — causing significant loss of passive and active range of motion, often preceded by a period of intense pain.

Shoulder Bursitis

Inflammation of the bursa — the fluid-filled sac that cushions the rotator cuff tendons — causing localized pain, swelling, and tenderness over the outer shoulder.

Calcific Tendinopathy

Calcium deposits that form within the rotator cuff tendons — often producing sudden, severe pain. Shockwave therapy is among the most evidence-supported treatments available for this condition.

Evidence-Based Findings on Shoulder Pain Treatment

Combining manual therapy with exercise outperforms exercise alone. Multiple studies confirm that a multimodal approach combining manual therapy with exercise produces significantly better outcomes than exercise therapy alone for rotator cuff-related shoulder pain — with improvements in both pain reduction and functional enhancement. (International Journal of Osteopathic Medicine, 2023.)

Shockwave therapy is highly effective for rotator cuff tendinopathy. A systematic review published in 2024 found that extracorporeal shockwave therapy (ESWT) produced a pain reduction SMD of 2.37 units for rotator cuff tendinopathy — a large and clinically meaningful effect size. ESWT is now considered a first-line non-surgical option for rotator cuff tendinopathy, calcific tendinopathy, and impingement syndrome.

Myofascial release reduces headache and disability in shoulder-related conditions. A 2024 systematic review and meta-analysis confirmed that myofascial release techniques significantly reduce pain intensity and disability in musculoskeletal shoulder conditions — supporting its role as a core component of integrative shoulder care.

Early conservative care prevents long-term complications. Evidence consistently shows that timely chiropractic and manual therapy treatment for shoulder injuries reduces the risk of scar tissue buildup, joint degeneration, and arthritis development — making early, targeted intervention far preferable to watchful waiting or symptom management alone.
